OFFICE HOURS: Monday – Thursday 8:30AM – 5:00 PM & Friday 8:30AM – 12:00 PM (lunch daily from 12:00 PM – 1:00 PM)
APPOINTMENT CANCELLATIONS: Patients are responsible for documenting and keeping track of scheduled appointments. A minimum of one business day is required to avoid being charged for a missed appointment.
PRESCRIPTION REFILLS:
(1) Please have all medication refill requests with you at the time of your appointment.
For prescription refills outside of your appointment time, please contact your pharmacy and request they send an electronic prescription refill request.
(2) Patients prescribed a Schedule II controlled substance (i.e. Adderall, Ritalin, Concerta, and Vyvanse) please contact our office and leave a detailed message including name, strength, and dosage of the medication you need refilled.
(3) Patients prescribed a Schedule II controlled substance are required to schedule regular med-check appointments every 3-6 months to continue medication refills.
AFTER-HOURS: Unless other arrangements have been made, please restrict pages to Dr. Larcade for urgent or emergency situations. Please do not use our after-hours answering service to request an appointment.
INSURANCE: Our office does not file insurance. However, we will provide you with the information needed to submit a claim with your insurance company to receive reimbursement directly from them.
PAYMENTS: Payment for services provided is due at the time services are rendered. We accept cash, checks, MasterCard, Visa, Discover and Amex. The fee for returned checks is $35.00. We request that you keep your account current.
HOLIDAYS/INCLEMENT WEATHER: Any changes to our normal office hours will be indicated on our main telephone greeting.
